    All new employees need to first have their fingerprints recorded or archived. If you have had your fingerprints scanned after 2/21/2003, please archive your Fingerprints by clicking here. If you have not yet had your fingerprints scanned or they were taken prior to 2/21/2003, please schedule your appointment through the New Jersey Department of Education's secure website by clicking here. See the link to the left titled “Fingerprinting for Public School Employment” for complete instructions regarding fingerprinting and the subsequent approval process through the State’s Criminal History Review Unit.


    Please print and complete the following forms and return your completed paperwork to Kate McEntee, Human Resources Generalist:

    1. Complete Employment Eligibility Verification Form I-9 (two forms of identification will be required

    2. Complete W-4 Form

    3. Complete Direct Deposit Form

    4. Complete Computer Internet Usage Agreement Form

    5. Complete Physical Examination Form

    6. Provide Proof of Mantoux Test (within 6 months)

    7. Complete Employee Health History

    8. Complete Emergency Contact Form

    9. Complete Pension Enrollment Form or the Pension Transfer Form (if coming from another school district)

    P.L 2018, c. 5 Sexual Misconduct/Child Abuse Disclosure

    Effective June 1, 2018, state statue (P.L. 2018, c.5) requires that all school districts, charter schools, nonpublic schools, and contracted service providers make certain inquiries regarding child abuse and sexual misconduct of prospective employees who will have regular contact with students. 

    All prospective employees must submit a Sexual Misconduct Child Abuse Disclosure Release Form to each of their former employers from the past twenty years to complete (only employers where there was direct contact with children).  Former employers must return completed forms to Kate McEntee in Human Resources within 20 days.
